Lesson Registration for 2011-2012

Instrument and Lesson Selection

Registration forms must be submitted by Monday, September 12th.

Please choose the instrument on which you would like to take lessons from the list below. If you are in Grade 4 or 5...you can only choose one from the following: flute, oboe, clarinet, alto saxophone, trumpet, French horn, trombone, and percussion (drum pad and bell set).

Note: If you are registering multiple students, please complete one form for each student. If your child is in middle or high school and will be studying more than one instrument in lessons, choose the primary instrument and tell us about the other(s) in the comments section at the bottom of this page.

Your Student's Lessons Note: All lessons must be paid in full before September 23rd and are non-refundable.

Lesson Options/Cost
You must indicate a first and second lesson option. Scheduling of semi-private and group lessons depends on the availability of other students. Although we do our best to place students in the preferred option, group lessons cannot be guaranteed due to numerous factors (day and time of preferred lesson, instrument chosen, teacher availability, etc.), therefore students may have to be placed in your second lesson choice.






Keep in mind, we cannot process your registration until your financial aid application is received. (Deadline September 9th)

Note: We will contact you immediately to let you know if there is an instrument available.

Preferred Lesson Location
Please check 'No Preference' or order ALL choices #1-#5. In most cases you will get your first or second choice.







Hours Available for Lessons
Because we schedule nearly 150 students, you MUST indicate at least 3 HOURS of weekly availabillity.
